For referring providers

Your patient, seen quickly and sent back documented.

We are an independent, physician-owned interventional pain practice in Abilene. All four of our providers are accepting new patients, and we see them by referral. Send the referral with recent records and imaging — we triage it the day it arrives, contact the patient to schedule, and send you a note after every visit and every procedure.

Make it fast

What to include

  • Demographics and insurance information
  • Reason for referral and duration of symptoms
  • Relevant imaging reports — MRI, CT, X-ray — and where the images live
  • Current medication list, including controlled substances
  • Prior treatments: therapy, injections, surgery, and their results
  • Note the office you are referring to, and attn: the provider if you have a preference

Incomplete referrals still get scheduled — we simply call your office for what is missing, which adds a day or two.

What happens next

Our commitment back to you

Receipt confirmed — your office hears from us, and your patient is contacted to schedule.

Consult note — findings, working diagnosis, and the plan, sent after the evaluation.

Procedure note — what was performed, the response, and next steps.

Medication clarity — we tell you plainly who is managing what, so prescribing never overlaps.

We also welcome patients who simply need a second opinion before committing to spine surgery — or a clear answer about whether their pain has an interventional target at all. See the full procedure list →
