This notice covers information collected through abilenepain.com. It is separate from the HIPAA Notice of Privacy Practices you receive at our office, which governs your medical record and your care.

Two different notices
Everything related to your diagnosis, treatment, and billing is protected health information under HIPAA. It is governed by our Notice of Privacy Practices, which we provide at your first visit and which you can request from either office at any time.
This page
The appointment request form, the education downloads, and normal web traffic are what this notice covers. A website form is not a secure patient portal, so we ask you to keep what you send here brief and non-clinical.
What we collect
If you use the appointment request form, we receive your name, phone number, email address, preferred office, the general area of your pain, and anything you write in the notes field. We only receive this when you press send.
Our hosting provider records standard technical information — IP address, browser type, pages requested, and timestamps — to deliver the site and protect it from abuse. We do not use it to identify individual visitors.
If you use the text size control, your choice is saved in your own browser so the site remembers it. That preference never reaches us. We run no advertising trackers, no social media pixels, and no third-party analytics.
To contact you about scheduling, and to answer the question you asked. That is the whole purpose.
We do not sell your information. We do not rent, trade, or share it for marketing. We do not use it to advertise to you elsewhere.
Only the vendors that make the site work: the company that hosts these pages and stores form submissions, and the service that delivers those submissions to our office email. They process the information on our behalf and are not permitted to use it for anything else.
We may disclose information if the law requires it, or if it is necessary to protect someone's safety.
Form submissions are kept only as long as needed to respond and, where you become a patient, to document that contact. If you would like a request deleted, email info@abilenepain.com and we will remove it.
Email, web forms, and text messages are not encrypted, and no one monitors them outside office hours. Do not use them for anything urgent.
If you are having a medical emergency, call 911. For an urgent clinical question during office hours, call 325-326-3433 and speak with our staff.
When you text or call us, standard message and data rates from your carrier apply. Texting us is your choice, and you can ask us to stop at any time.
Where we link to another organization, that site has its own privacy practices and we are not responsible for them.
This site is intended for adults. We do not knowingly collect information from children through it.
If we change how this site handles information, we will update this page and revise the effective date above.
