Before your first visit
New patient forms
New patients are generally seen by referral — ask your primary care provider or specialist to fax a referral with your recent records and imaging to 325-378-9175, and we will call you to schedule. Our office sends your new patient paperwork before your appointment — text or call us any time if you would like it sooner. Completing it early helps our team understand your history, previous treatments, medications, and goals for pain relief, but if that is not possible, we will have printed forms ready for you to fill out when you arrive.

Bring a photo ID, your insurance card, a current medication list, and any recent imaging reports.
Insurance · We accept most major plans, including Medicare, TRICARE, and commercial insurance. We do not accept Medicaid. Call us to verify your specific plan before your visit.

The portal is HIPAA-compliant and available 24/7 from your phone, tablet, or computer. If you need help logging in or resetting your password, call the office and we will be glad to assist.

All the features of the portal on your phone, with push notifications: appointment reminders and check-ins, forms, secure messaging, insurance cards and photos, test results and treatment notes, telehealth visits, medications and refill requests.
Search "APPatient" in the App Store or Google Play, or ask our staff to send you the link.
Klara — reminders & forms by text
Your appointment time should go to your pain relief, not paperwork in the waiting room. Klara lets us communicate with you directly by text message — no app to download and no login to remember.
